The DA Form 3643 records daily petroleum product issues across all military service branches. It captures the type, grade, and quantity of fuel distributed to each unit. The issuing station, vehicle registration number, and receipt signature are all logged on this one-page form.
Army Regulation 703-1 (AR 703-1) authorizes the use of this document. The Deputy Chief of Staff for Logistics (DCSLOG) oversees its administration across the Department of Defense. All four service branches use it: Army (A), Air Force (AF), Navy (N), and Marine Corps (M).
The current version was revised in April 1985. It replaced an earlier edition that had fewer fields for tracking vehicle and unit data. The revision added distinct columns for vehicle registration numbers and receipt acknowledgments, improving petroleum accountability.
Petroleum types commonly documented include JP-8 jet fuel, diesel, MOGAS (motor gasoline), and aviation fuel. Each entry records the quantity in the applicable unit of issue. Completed forms provide a permanent audit trail for military fuel distribution. For vehicle-level tracking, see the Individual Vehicle Distance Fuel Report.
