DA Form 4986 at a glance
What it is: Recording identifying data of your high value property will assist in recovering lost or stolen property, and for claims against the Government and/or private insurance, as appropriate.
Who signs: The owner signs under SIGNATURE OF OWNER. In the SIGNATURE OF INDIVIDUAL VERIFYING RECORDING OF PROPERTY AND DATE box, the individual verifying the recording of the property signs and dates.

Pages and date of preparation
Fill in PAGE NO., NO. OF PAGES and DATE OF PREPARATION.
Owner name, DOD ID number and signature
Enter the TYPED OR PRINTED NAME OF OWNER (Last, First, MI) and the DOD ID NUMBER.
The owner signs under SIGNATURE OF OWNER.
Item number, name, QTY and description
For each item, enter the ITEM NO., NAME OF ITEM and QTY.
Enter the BRAND NAME, MODEL OR STYLE, OTHER DESCRIPTION.
Serial number or marking, date, value and verifying signature
Enter the SERIAL NO. OR MARKING.
Enter the DATE AQUIRED, if known, and the VALUE.
In the SIGNATURE OF INDIVIDUAL VERIFYING RECORDING OF PROPERTY AND DATE box, the individual verifying the recording of the property signs and dates.

When marking your personal property, use the prefix “USA” followed by your DOD ID Number.
Keep this record of high value property in a safe place and accessible.
DISCLOSURE: Voluntary. However, failure to provide information will have no adverse effect on the individual.
