What Is DD Form 2648?

The Preseparation Counseling Checklist (DD-2648) is a Department of Defense document. It is required for service members separating from active duty. This includes Active Component (AC), Active Guard Reserve (AGR), Active Reserve (AR), Full-Time Support (FTS), and Reserve Program Administrator (RPA) personnel. All U.S. military branches use this checklist.

The government requires this pre-separation checklist at least 90 days before a member's anticipated separation date. The goal is to ensure all personnel know about available transition assistance. These services and benefits cover six key areas:

• Career guidance and employment assistance
• Educational and training benefits (including GI Bill options)
• Healthcare coverage during and after the transition
• Relocation assistance and housing support
• Financial planning and retirement options
• Legal assistance and family support services

Service members separating from active duty benefit most by starting this checklist early. Early completion allows time to arrange each recommended service. It also allows time to ask questions before the separation date arrives.

Completing the checklist triggers the creation of an Individual Transition Plan (ITP). The ITP is a personalized roadmap for civilian life. It reflects the member's goals, timeline, and support needs. Department of Defense issuances require all eligible members to complete both the checklist and the ITP on time. Reserve Component members should also review the companion DD Form 2648-1 for Reserve and Guard requirements.
