Use FormsPal to access and download DD Form 175-1. Before you start filling out the form, gather the following information from your aviation weather forecaster or official weather products:
- Planned takeoff time in local and Zulu (UTC) formats
- Departure runway designation
- Takeoff temperature and dewpoint at the departure airfield
- Pressure altitude for the departure location
- Surface wind direction in degrees magnetic and speed in knots
- Climb winds for the initial departure altitude
- Flight level winds and temperatures at each cruise altitude
- Active SIGMET and AIRMET advisories along the route
- Terminal Aerodrome Forecasts (TAFs) for destination and alternate airfields
Step 1 – Fill Out Part I (Takeoff Data)
Start at the top of the form. Write the planned takeoff time in local time, then enter the Zulu (UTC) equivalent in the adjacent field. Enter the two-digit runway designation and the direction the aircraft will be heading at takeoff. Record the current takeoff temperature in degrees Celsius and the dewpoint reading from the current weather observation. Enter the pressure altitude for the departure airfield in the designated box. Fill in the surface wind direction as a three-digit magnetic bearing and the speed in knots. If your weather briefer has provided climb winds for the initial departure altitude, enter that wind direction and speed in the climb wind fields in the lower portion of Part I.
Step 2 – Fill Out Part II (En-route and Mission Data)
Part II covers weather along the planned flight route. Start with the flight level winds and temperatures section in the upper portion of the form. Using the most current WINDS ALOFT forecast, record the wind direction and speed plus the temperature in degrees Celsius at each applicable altitude block for your planned route. Work through the altitude levels from low to high and enter only the levels that apply to your mission profile.
Next, work through the weather hazard checkboxes from top to bottom. For turbulence, check the applicable severity box (LIGHT, MODERATE, SEVERE, or EXTREME) and enter the location and valid altitude range in the adjacent fields. For icing, mark the forecast type (RIME, CLEAR, or MIXED) and the altitude band where icing is expected. Enter the forecast freezing level altitude in feet MSL in the FREEZING LEVELS field. Check any applicable precipitation types, such as SHOWERS or FREEZING rain, and note the geographic location and valid times for each hazard. If active SIGMETs or AIRMETs cover your route, record the advisory numbers and a short description of the covered area and altitudes.
Step 3 – Fill Out Parts III, IV, and V
Part III requires Terminal Aerodrome Forecast (TAF) data for your destination airfield and at least one alternate. For the destination, enter the expected surface wind direction and speed, the lowest forecast cloud layer ceiling height in hundreds of feet and the sky coverage code (FEW, SCT, BKN, or OVC), the forecast visibility in statute miles, any significant weather phenomena using standard aviation codes (TS, FG, RA, SN, etc.), and the altimeter setting in inches of mercury. Record the valid time for each TAF in the column provided. Repeat the process for each alternate airfield, using the TAF valid during the planned arrival window at that field.
Part IV is for additional information not captured in the structured sections. Write the reference numbers and titles of any attached weather products, PIREPs, or special mission advisories in the REMARKS and ATTACHMENTS fields. If the briefer has verbally added information that is not on a standard form, note a brief summary here. Mark the ATTACHMENTS checkbox YES if any supplementary products are included.
Part V is the sign-off section. The aviation weather forecaster enters the date the briefing took place, the local briefing time, and the void time after which this briefing is no longer valid for planning. The FLIMSY BRIEFING NO field gets the serial number assigned to this transaction. The forecaster then prints their initials and signs in the FORECASTER'S INITIALS field. The pilot, navigator, or other crew member receiving the briefing prints their name in the NAME OF PERSON RECEIVING BRIEFING field and countersigns to confirm they received and understood the complete weather briefing.
Review all fields before signing. The void time in Part V must match the expiration time shown on the weather products used. Both the forecaster and the receiving crew member must sign before the form is filed. Retain one copy with the flight records and give a copy to the pilot.
