MV-11: Pennsylvania Application for Permanent Antique, Classic or Collectible Registration Plate

Check the block to indicate the type of registration plate desired: ANTIQUE, CLASSIC or COLLECTIBLE.

For COLLECTIBLE, the vehicle must be currently branded as a collectible vehicle or the vehicle must follow procedures as outlined in Form MV-426B, “Application for Reconstructed, Specially Constructed, Collectible, Modified, Flood, Recovered Theft Vehicles and Street Rods.”

There is a fee required for any of the antique, classic or collectible registration plates, plus a certificate of title fee and a fee for local use, if applicable. The use of antique, classic and collectible registration plates is governed by Section 1340 of the Pennsylvania Vehicle Code which states: “It is unlawful for any person to operate a motorcycle or vehicle with antique, classic, vintage, collectible or historic military vehicle registration plates for general daily transportation. Permitted use shall be limited to participation in club activities, exhibits, tours, parades, occasional transportation and similar uses.” Occasional transportation and similar uses is defined as no more than one day a week. If you are applying to display a vintage registration plate on your antique or classic vehicle, you must complete Form MV-11V, “Application to Display a Vintage Registration Plate,” and submit with appropriate fees. Collectible vehicles are not eligible to display a vintage registration plate. Antique and Classic registration plates may not be displayed on a vehicle used to transport people or property.

If your title is being held by a lienholder and it is an Electronic Lien Title (ELT), check here. No Paper Title Will Be Attached.

Under APPLICANT(S) INFORMATION, enter the Last Name (or Full Business Name), First Name, Middle Name, PA DL/Photo ID# or Bus. ID# and Date of Birth.

For a Co-Owner, enter the Last Name, First Name, Middle Name, PA DL/Photo ID# and Date of Birth.

Enter the Street Address, City, State, Zip Code and Daytime Telephone Number.

Under VEHICLE DESCRIPTION AND GENERAL INFORMATION, enter the Title Number, Vehicle Identification Number, Make of Vehicle, Model Year and Body Type.

Enter the Insurance Company Name, Policy Number, Policy Effective Date and Policy Expiration Date. A copy of valid Proof of Insurance must be attached.

Under OPTIONAL PERSONALIZATION REQUEST, an additional fee is required. Enter a FIRST CHOICE, SECOND CHOICE and THIRD CHOICE. Please use capital letters and print clearly. No other special characters are available.

For Antique registration plates, up to FOUR letters and/or numbers in combination. For Classic registration plates, up to FIVE letters and/or numbers in combination. For Collectible registration plates, up to FOUR letters and/or numbers in combination. On Antique, Classic and Collectible registration plates, if a hyphen or space is used as part of the registration configuration, it counts as one of the available spaces for personalization. Only one hyphen or space is permitted, but not both. A pre-printed letter configuration of “C” will precede your personalized configuration on your Classic registration plate and cannot be changed. A pre-printed letter configuration of “CV” will precede your personalized configuration on your Collectible registration plate and cannot be changed.

For Antique Motorcycle registration plates, up to FIVE letters and/or numbers in combination. For Classic Motorcycle registration plates, up to FOUR letters and/or numbers in combination. For Collectible Motorcycle registration plates, up to THREE letters and/or numbers in combination. On Motorcycle registration plates, if a space is used as part of the registration configuration, it counts as one of the available spaces for personalization. Only one space is permitted. No hyphen is available. A pre-printed letter configuration of “CL” will precede your personalized configuration on your Classic Motorcycle registration plate and cannot be changed. A pre-printed letter configuration of “CM” will precede your personalized configuration on your Collectible Motorcycle registration plate and cannot be changed.

Allow 8 to 10 weeks for delivery.

Under SELF-CERTIFICATION AND SIGNATURE, sign on the Signature line, have a co-owner sign on the Signature of Co-Owner/Title of Authorized Signer line, and enter the Date.

The vehicle has been maintained in or restored to a condition that is essentially the same as that which a vehicle of that particular year and model would have been in at the time it was originally manufactured and both interior and exterior are in good condition including paint, chrome, wheels and body. The signature(s) in part E hereby certifies under penalty of law that the information contained herein is TRUE AND CORRECT. WARNING: Unsworn falsification to authorities is a misdemeanor of the third degree punishable by a fine of up to $2,500 and/or imprisonment up to 1 year (18 Pa.C.S. Section 4904[b]) (checked October 20, 2026).

Under COLLECTIBLE VEHICLE INFORMATION, describe in detail how the vehicle has been modified to qualify as a collectible vehicle. All engine and exhaust system modifications must be described in detail and two additional color photographs are required. One photo of the engine compartment that shows the qualifying engine modifications and the other of the exhaust system that shows the modified changes or the new exhaust system that was installed must be included. If the change is not engine related, i.e., low slung “1950's” type vehicles, an additional color photograph of the vehicle must be included. Please use additional pages if more space is required.

If the vehicle is already branded as a collectible vehicle, complete Form MV-11. If your vehicle is not currently branded as a collectible vehicle, you must follow the procedures outlined on Form MV-426B.

For Collectible registration plates, Pennsylvania Certificate of Title (Form MV-4) and Vehicle Sales and Use Tax Return/Application for Registration (Form MV-4ST), or out-of-state title and completed Application for Certificate of Title (Form MV-1) are required to be submitted when the vehicle is being transferred to a new owner and the new owner is making application for a collectible plate.

The appropriate sales tax, title/registration fees and four clearly distinguishable color photographs of the vehicle (front, rear and both sides). Two additional color photographs are required when the engine has been modified, one of the engine compartment that shows the qualifying engine modifications and the other of the exhaust system that shows the modified changes or the new exhaust system that was installed.
