The fee is $100.00, payable to the Secretary of State (checked October 19, 2026).

Check the box if rush is requested; the additional fee is $10.00 (checked October 19, 2026).

Enter the first owner's name (first, middle, last) in 1a and the date of birth in 2a. If more than one owner, common ownership will be assumed unless the Joint ownership box is checked ("or" is not permitted).

If there is a second owner, enter the second owner's name (first, middle, last) in 1b and the date of birth in 2b. Enter the telephone number in 3.

In 4, enter the mailing address: Street / PO Box, City, State and ZIP.

In 5, enter the legal address (if other than the mailing address): Street / PO Box, City, State and ZIP.

Enter the Year in 6, the Make in 7, the Model in 8, the Serial Number or Vehicle Identification Number in 9 and the Body Type in 10. In 11, enter New or Used.

Enter the Purchase date (Mo/Day/Yr) in 12, the Previous title number in 13 and the State of origin in 14.

A lienholder is the institution or person financing the manufactured home or tiny home. The title will be mailed to the first lienholder; if "none", it will be mailed to the owner.

In 15, enter the first lienholder's name; if no lien, state "none". Enter the First Lienholder ID# and, in 17, the Date of 1st lien (Mo/Day/Yr).

If there is a second lienholder, in 18 enter the second lienholder's name and the Second Lienholder ID#; in 20, enter the Date of 2nd lien (Mo/Day/Yr).

In 16, enter the first lienholder's mailing address, and in 19 the second lienholder's: Street / PO Box, City, State and ZIP.

In 21, enter the Name of seller; in 22, enter the Address of seller (Street / PO Box, City, State, ZIP); in 23, enter the Maine dealer plate or license #.

The Maine Dealer's Representative must sign in 24 and enter Name, Official Title, Date and Telephone.

The dealer's representative certifies that the manufactured home or tiny home described was sold to the owner(s) named and that the information shown is true and correct to the best of the representative's knowledge, information, and belief.

The undersigned certify that all liens against the manufactured home or tiny home described are listed and that the information contained herein is true and correct to the best of their knowledge and belief. If under age 18, the Parent or Legal Guardian must also sign; state the relationship. If representing a company, the undersigned further certify that they have been authorized by the company to sign on their behalf. Power(s) of attorney must accompany this form.

A person who uses a false or fictitious name or address, makes a materially false statement, fails to disclose a security interest, or conceals any other fact on an application for certificate of title is guilty of a Class D crime.
