What it is: The South Carolina Department of Motor Vehicles Change of Residency Certification, DL-222: as an out-of-state resident, you may request an out-of-state waiver for Form SR-22 to reinstate your driving privileges.
Who signs: Sign under Customer's Signature and enter the Date.
This form must be completed by an individual who lives in a state other than South Carolina (SC); and is trying to get a driver's license or beginner's permit in that state; and has a driving suspension from SC that requires proof of insurance (Form SR-22).

How to fill out Form DL-222, box by box
Section I, Name of Customer
Section I is to be completed by the customer. Enter the Name of Customer.
Enter the SC Driver License/BP/ID No and the Date of Birth.
Enter the Customer's Home No, the Cell Phone No and the Work Phone No.
Change of residency: I became a resident in the state of
Under penalties of perjury, you declare that the following facts concerning your change of residency from South Carolina are true and correct.
In box 1, enter the day, the blank after day of, the number after 20 and the state in which you became a resident.
Enter your current address.
I applied for a driver's license in the above referenced state
In box 2, enter the day, the blank after day of and the number after 20 on which you applied for a driver's license in the above referenced state.
I request a waiver for the Form SR-22 filing
Box 3: you do not wish to keep an SC Driver's License and you request a waiver for the Form SR-22 filing.
More rules for Form DL-222
Section II is to be completed by a DMV Official of the customer's new state of residence, who verifies that the named individual has, in fact, applied to receive a license in that state. Once the SCDMV has received this verification, it will certify that his or her SC driver's license is not under suspension and then he or she will be eligible for a license in that state.
Section III is to be completed by a South Carolina Department of Motor Vehicles (SCDMV) employee: Form SR-22 filing requirement has been waived.
