How to Fill Out College Application Form

Name the college and specify the start of studies

First of all, fill out the name of the college you are applying to, the time when you wish to begin the term, and your possible major at that college. On the right side of the page, mark only one box that is appropriate for your case, also write about your future possible career plans;

Define payments

In the next section, one must indicate data about payment. Choose if you would like or would not like to apply for a counselor-approved fee waiver, mark either ‘Yes’ or ‘No.’ Also, indicate if you are applying for financial help or not, also mark ‘Yes’ or ‘No.’ In case you do, write out when did you or will you file the needed forms required for financial aid;

Fill out personal data

Next up is the ‘Personal information section. There are several subsections, and the first one is about general personal data:

• One must write out the full legal name as written on passport or ID, as well as the date of birth;
• Also, indicate your legal sex, and, if needed, one can mark the gender identity;
• One must include social security number, and if there is any, it is optional;
• One can also write out a preferred name, and it is compulsory to indicate any previous names if there are any;
• You also will have to leave your e-mail address and indicate marital status.

The next thing on our list is citizenship:

• Write out your full place of birth: country, city, state, or province;
• You should also mark the appropriate box beneath. The boxes are connected with citizenship. If you are a dual US citizen, you must specify your other citizenship. If you have a US permanent resident visa, write out your citizenship. Also, indicate the alien registration number. In case you are a citizen from some other country, mark the data about your Visa;
• If you are not a citizen of the US, but you do live in the United States, you should indicate the number of years you have been living here;
• Indicate your first language or language you speak in your home if it is not English.

The following subsection is connected with ethnicity:

• Indicate if you are a Hispanic or Latino. If you are, enter your country of family’s origin;
• Then, below choose a box that best describes your racial background.

Add info concerning your family

The next section is connected with information about the family of the applicant. First, write out the information about the first parent or guardian (mark the appropriate):

• Indicate the full name of the person, as well as the sex;
• In case the parent or guardian has a living – indicate it. If no, write out the date of decision;
• If the address of the parent or guardian is different from yours, indicate the full address, cell-phone number, e-mail address, profession, job position, and employer’s name. If the parent or guardian attended the college, indicate which one exactly, write out the degree earned and date of graduation. If the parent or guardian attended the graduate school, indicate which one exactly, write out the highest degree earned and year of graduation.

Secondly, indicate the very same information about the second parent or guardian.

Finally, fill out some other data concerning the family:

• Write out the marital status of your parents. If they are divorced, enter the date of the divorce;
• Mark with whom do you live, choose the appropriate box;
• In case you have siblings, enter the full names, legal sexes, ages, college (if any), degree(s), and dates of attendance.

Provide academic information

The next part is about ‘Academic information.’ First, fill out some general background data:

• Enter the name of your school and the CEEB code. Mark the type of school you attended, choose one appropriate option;
• Fill out the full address of the school, the date of your school enrollment, and the date when you finished school;
• You must also write about the name and e-mail address of your school counselor, as well as the phone number and fax of the school;
• If you are currently enrolled in school, mark it in the next step. If you did or will graduate from High School early, also mark it;
• In case you have received a GED – mark it in the next step. If you marked ‘Yes,’ list the official date and attach the official scored from the testing agency;

The next subsection is dedicated to the current year’s courses:

• You will have to write out the name, level (Honors, AP, IB, and so forth), and amount of credits value of your current courses. You should put the data in the columns Semester 1 and Semester 2, or Trimester 1, Trimester 2, and Trimester 3.

In the following subsection, mention all of the schools that you have attended:

• First of all, mention all of the high schools, include such data as the name of the high school, CEEB code, dates in which you attended the school, and the school’s location;
• Last, mention all of the colleges and universities, include such data as the name of the college or university, CEEB code, dates in which you attended the educational organization, and its location.

Provide AP and IB test scores

The next part is about AP and IB test scores. One will have to mention any Advanced Placement (AP) or International Baccalaureate (IB) examinations. Please, mention test dates, subjects, and scores.

Provide Standardized Test data

The following section is connected with Standardized Test Information. You should include all necessary information from the official testing agencies:

• For SAT Reasoning include such data as test date, scores for Math, Optional Essay and Evidence-Based Reading and Writing;
• For SAT Subject mention test date, subject taken, and score;
• For ACT, include such information as test date and scores for Math, English, Reading, Science, Composite, and Optional Essay;
• For the TOEFL exam, mention the test score, subject, and test date.

Add info about honors

The next part is about academic or educational awards and honors one has received in high school. In case you had any, attach your response to your application

Provide info about volunteering

The following section is called ‘Extracurricular and Volunteer Information.’ Please write out all the crucial and meaningful extracurricular or any other activities you have taken part in, list your special achievements. To be more precise, write out the activity type, the grade in which you have taken part in that activity, special achievements, amount of time you have spent on this activity. Also, indicate if you will or will not (or if you are unsure) participate in the activities in College.

Describe employment issues

The next part is connected with employment issues:

• List all of your jobs and work experience. Include such data as your employer, work description, dates of employment, and hours per week working time;
• Also, indicate if you are a Veteran, Dependant of US Veteran, Active US Military, National Guard, or Active Reserve. Mark the applicable;
• If you are or were part of the military, specify which part exactly, mark the needed box;
• Indicate if you plan or not to use Veteran Benefits;
• Indicate if you were or were not honorably discharged from some kind of military of the US. If you’re not, explain yourself in the additional data section.

Add activity description

Next, there is a part about the activity description. Write 100-150 words or attach your activity description to the application.

Write a Personal Statement essay

Afterward, you will have to complete the Personal Statement essay. It should be of 650 words or fewer, and it should show how well do you formulate your thoughts and how you communicate.

Provide multimedia info

‘Multimedia Information’ is the next section. This is an optional part, where you can provide your link to some video or image or basically anything that demonstrates your skills or achievements. Below the link, please briefly describe what is on the link you provided

Provide additional info

In the ‘Additional Information’ part, write out about anything you wanted but didn’t mention throughout the application.

Provide discipline data

The next section is connected with discipline data:

• Indicate if you have ever been suspended, removed, dismissed, or expelled from any education organization starting from 9th grade;
• if you have ever committed any crime except for traffic offenses, also indicate it.

If you have answered ‘Yes’ to any of the questions above, please provide the dates on which you have committed something and explain it in detail.

Authorize the paper

The final step is ‘Authorization.’ Read every part of the document carefully. Then just place your signature and put the current date.
