The AD-435A is a ten-page USDA performance appraisal document. Follow the steps below to complete each section correctly.
- Employee and Position Information: Enter the employee name, position title, pay plan, grade, step, and the start and end dates of the rating period.
- Critical Elements: List each critical element of the employee role. For each element, write a performance standard that describes what a Fully Successful result looks like. Critical elements must be documented before the rating period begins.
- Non-Critical Elements: Add any non-critical elements relevant to the position. These elements are considered during the appraisal but do not independently determine the summary rating.
- Progress Review: At the mid-point of the rating period, the supervisor meets with the employee to discuss progress. Document the review date and have both parties sign.
- Annual Appraisal: At the end of the rating period, the supervisor assigns a rating for each element. Add narrative comments to support ratings that are above or below Fully Successful.
- Summary Rating Calculation: Calculate the overall summary rating based on element ratings. At least one critical element rated Unacceptable results in an Unacceptable overall rating.
- Certification and Signatures: The supervisor, reviewing official, and employee all sign and date the completed form.
What is the difference between the AD-435 and AD-435A forms?
The AD-435 form is a shorter version used for some USDA positions. The AD-435A form includes additional sections for non-critical elements and is used for most permanent USDA positions that require a full appraisal worksheet.
