The AF 3994 form (also called the AF IMT 3994 or AFCENT Form 3994) is the official Air Force document a commander uses to nominate personnel for decorations earned during deployment or contingency operations. It covers acts of valor, meritorious service, and heroism that occur outside routine peacetime assignments.
The two-page form includes five main components: administrative data identifying the operation name and inclusive dates, the nominee's personal information including rank and AFSC, the decoration being recommended, a narrative citation summarizing the qualifying actions, and endorsement signatures from the chain of command. Commanders may check a box to recommend adding a "V" Device to the decoration for valor under direct combat. Multiple nominees can be recognized on a single form when the action involved a group.
For Air Force personnel, the AF 3994 is one of several official recognition forms. The AF Form 1206 (Nomination for Award) handles peacetime and unit-level nominations. Personnel managing home-station matters during deployment may need a Military Power of Attorney. Unit commands that issue separate recognition documents can use the Award Certificate template for unit-level acknowledgments.
