By completing this form accurately, breeders satisfy the AKC's requirement to retain litter documentation for at least five years. Each litter entry includes the dam's registered name and AKC number, the sire's registered name and AKC number, the date of birth, and the total count of male and female puppies born. These top-level fields establish the identity of the litter within AKC records.
At the individual puppy level, the form records the AKC litter number assigned to each dog, the new owner's full name and address, the date when ownership papers were transferred, and any microchip or tattoo identification applied for permanent ID. Sex, color, and markings are also noted for each puppy to support future breed verification.
Breeders who work with multiple breed registries may also need the ACA Litter Registration Form or the ICA Litter Registration Form for puppies registered with other canine organizations.
