The Department of the Army has released DA Form 3034, also known as the "Army Maintenance Management System (Ammo)" form. This new form is used to report and manage ammunition conditions and performance data. The purpose of this form is to improve the accuracy of information reported on ammunition status and provide a more comprehensive view of Army ammunition inventories. It replaces previous forms such as DA Form 843 (Ammunition Expenditures) and DA Form 3349-R (Ammunition). The new DA Form 3034 will be effective October 1, 2017.
