Complete and submit this form if your public lodging establishment is 3 or more stories in height. The inspector must have the education and experience to be competent to perform the inspection. The operator is responsible for verifying the competency of the inspector.

How to fill out DBPR HR-7020, box by box
Section 1, Owner Name, License Number and Mailing Address
For new or renewal inspections complete sections 1 and 2.
Section 1, Establishment Information, is for new or renewal only: enter the Owner Name and the License Number.
Enter the Mailing Address, then the City, State and Zip Code.
Section 1, Establishment Name (DBA), Establishment Address and Telephone Number(s)
Enter the Establishment Name (DBA) and the Establishment Address, then the City, County and Zip Code.
Enter the Telephone Number(s).
Section 2, Inspection: areas, defects and repairs
Section 2, Inspection, is for new or renewal only. Section 2 certifies that any and all balconies, platforms, stairways, railings and railways on the above-described premises were inspected on _________ by a person competent to conduct such inspection, and were found by such person to be safe, secure and free of defects. Enter the date of inspection on the "were inspected on _________" line.
The term “balcony” means “a landing or porch that is accessible to or used by the public…” (Section 509.2112, F.S. and Rule 61C-3.001(5)(a), F.A.C.)
The balcony inspection must include platforms, stairways, railings and railways, guardrails, balustrades, parapets, and areas enclosed by screening or other non-permanent building material.
Enter the Total Number of Areas Inspected, the Total Number of Defects Found and the Date Repairs Completed.
Section 2, Inspector and Signature of Inspector
Enter who conducted the Inspection. For new or renewal only, in Section 2, on the line below "who is competent to conduct such inspections because:", provide facts/credentials establishing competency.
The inspector signs on the Signature of Inspector line and enters the Date.
Section 2, Name of Operator and Signature of Operator
Enter the Name of Operator. The operator signs on the Signature of Operator line and enters the Date.
Section 3, Management Information: change of owner only
Section 3, Management Information, is for change of owner only: enter the Name of Operator and the Date, and the operator signs on the Signature of Operator line.

Common questions about DBPR HR-7020
Is there a deadline?
This Certificate of Balcony Inspection expires three years from the date of inspection listed in Section 2 of this form or from the date of inspection listed in Section 2 of the attached previous owner's form. A new Certificate of Balcony Inspection must be completed and submitted prior to the expiration date.
