How to fill out DD Form 2522, box by box
Boxes 1 to 5: the chemical and the item
In Box 1, enter the chemical/common name.
Enter the hazard code in Box 2, the NSN/LSN in Box 3 and the part number in Box 4.
In Box 5, enter the item name.
Box 7: specific hazards and precautions
In Box 7, enter the specific hazards and precautions, including target organ effects. See MSDS for further information.
Box 8: protect
In Box 8, under PROTECT, X all that apply: EYES, SKIN or RESPIRATORY.
Box 9: contact company and Box 10: procurement year
In Box 9a, enter the name of the contact company.
In Box 9b, enter the address: Street, P.O. Box, City, State, ZIP Code, Country.
In Box 9c, enter the emergency telephone number, including the area code.
In Box 10, enter the procurement year for the hazardous chemical.
More rules for DD Form 2522
In Box 6, X all that apply under HAZARDS for HEALTH, CONTACT, FIRE and REACTIVITY: (1) ACUTE (Immediate) and (2) CHRONIC (Delayed), with NONE, SLIGHT, MODERATE or SEVERE.
The previous edition may be used.
