DD Form 2506 at a glance

How to fill out DD Form 2506, box by box
The vehicle
Under 1. VEHICLE IDENTIFICATION, enter the MAKE, MODEL, YEAR, COLOR and VEHICLE IDENTIFICATION NO.; under VEHICLE LICENSE, enter the (1) NUMBER, (2) STATE and (3) YEAR, then the MILEAGE and DECAL NO.
Registered owner
Under 2. REGISTERED OWNER, enter the NAME (Last, First, Middle Initial), the ADDRESS (Street, Apartment Number, City, State and ZIP Code), the ORGANIZATION and the TELEPHONE NUMBER (Include Area Code).
Vehicle operator
Under 3. VEHICLE OPERATOR, enter the NAME (Last, First, Middle Initial), the ADDRESS (Street, Apartment Number, City, State and ZIP Code), the ORGANIZATION and the TELEPHONE NUMBER (Include Area Code).
Reason for impoundment
Under 4. REASON FOR IMPOUNDMENT, X all that apply: Accident, Burned, DWI, Abandoned, Illegally Parked or Stolen; for any other reason, mark Other (Specify).
Damage to vehicle
Under 5. DAMAGE TO VEHICLE, a. SHADE DAMAGED AREA OF VEHICLE, then b. X ALL THAT APPLY: mark Intact or Missing.
Condition of vehicle when impounded
Under 6. CONDITION OF VEHICLE WHEN IMPOUNDED, X all that apply: Door Locked, Door Unlocked, Trunk Locked, Trunk Unlocked, Keys in Car or Keys Missing; for any other condition, mark Other (Specify).
Location, condition, property and remarks
Enter the LOCATION OF VEHICLE in 7, the CONDITION OF VEHICLE in 8, the PERSONAL PROPERTY CONTAINED IN VEHICLE in 9 and the REMARKS in 10; for boxes 8, 9 and 10, attach additional pages if more space is needed.
Date and time impounded, towed at and stored at
Under PART III - DISPOSITION, enter the DATE IMPOUNDED as YYYYMMDD, the TIME IMPOUNDED, TOWED AT and STORED AT.

Under 13. REPORTED BY, enter the NAME (Last, First, Middle Initial), RANK, DATE and ORGANIZATION, then add the SIGNATURE.
Witnessed by
Under 16. WITNESSED BY, enter the NAME (Last, First, Middle Initial), RANK, DATE and ORGANIZATION, then add the SIGNATURE.
Released by
Under 17. RELEASED BY, enter the NAME (Last, First, Middle Initial), RANK, DATE and ORGANIZATION, then add the SIGNATURE.
