What it is: Form MV-41 (1-26) is the Application for Correction of Vehicle Record or Verification of Vehicle Identification Number.

What to bring and attach to Form MV-41
If the 1st block (Correction or Verification of Vehicle Identification Number) is checked on reverse, certificate of title and title fee must be attached. Certificate of title and fee are not required when used for verification of VIN only.
If the 2nd block (Application for State Replacement Vehicle Identification Number Plate/Decal) is checked on reverse, certificate of title and title fee must be attached.
If the 3rd block (Correction of Body Type, Number of Axles, or Other Corrections) is checked on reverse, certificate of title and title fee must be attached. Current certificate of title is not required when applying for a correction of the registered gross weight and there have been no alterations to the vehicle. Certificate of title and title fee must be attached for all other corrections.
In addition to the title fee (if applicable), appropriate increase fee for the difference in registration weight and seating capacity must also be attached if registration is desired.
Current certificate of title must be attached when applying for a correction of the odometer reading. In addition, copies of past 2 years inspection records or repair bills that cover the time of the odometer discrepancy must be attached.

How to fill out Form MV-41, box by box
Registration Plate Number, Title Number and lienholder
Enter the Registration Plate Number and the Title Number. Mark YES or NO for "Is lienholder an ELT Participant?" Enter the Financial Institution Number.
If the title is being held by a lienholder that participates in the Electronic Lien and Titling Program (ELT), there will be no title attached.
Check the applicable block
Check the applicable block. For Correction or Verification of Vehicle Identification Number, complete Sections A, B and E.
For Application for State Replacement Vehicle Identification Number Plate/Decal, complete Sections A, B (must be completed by a Police Officer) and E. Original Vehicle Identification Number (VIN) will be used on state replacement VIN plate unless original number cannot be determined. A corrected title and registration card will be issued indicating that a VIN plate has been replaced.
For Correction of Body Type, Number of Axles, or Other Corrections to the Vehicle Title or Registration Data, complete Sections A, C and E.
For Verification of Gross Vehicle Weight Rating (GVWR) or Gross Combination Weight Rating (GCWR) by Inspection Mechanic Only, complete Sections A, D and E.
Vehicle owner information
Enter the vehicle owner's Last Name (or Full Business Name), First Name, Middle Name, Date of Birth, and PA DL/Photo ID# or Bus. ID#.
Individuals should list their PA Driver's License (PA DL) or Photo ID# in the space provided. Businesses should list their Business ID# (Bus. ID) where indicated (i.e. E.I.N.).
If there is a co-owner, write the Co-Owner Last Name, First Name, Middle Name, PA DL/Photo ID# and Date of Birth.
Fill in the Current Street Address, City, State and Zip Code.
In County Code, use the County Codes listing found on the back of the form and list the numerical code for your county of residence.
Correction or Verification of Vehicle Identification Number
To be used only to correct typographical error recorded as the VIN or for the purpose of VIN verification. This application can only be used to correct a typographical error of one digit, transposition of two digits, or if the vehicle's engine number has been erroneously recorded as the Vehicle Identification Number (VIN).
Attach tracing in Section B, or have an authorized notary public employed by a full agent or motor vehicle dealer, or inspection mechanic complete the verification.
Check Correction, Verification, Lost, Stolen, Defaced or Never Received, or check Other and explain.
Record the Correct VIN, and enter the Original Vehicle Identification Number (if known).
Verification by Police Officer, Notary Public or Inspection Mechanic
Verification of the VIN can be done by authorized vehicle safety inspection mechanics and notaries public employed by a full agent or motor vehicle dealer; however, agents, inspectors, and law enforcement personnel are prohibited from verifying the VIN of their own personal vehicles.
For Verification by Police Officer (Local or State Police): Section B on the front of the form must be completed by a police officer, if the police officer is verifying the original Vehicle Identification Number (VIN). The police officer must sign their name and list their badge number, department name, and telephone number.
The Notary Public or Inspection Mechanic signs and enters the DIN/Mechanic #.
Vehicle data that requires correction
Check only the block(s) and list the information that needs correcting due to an error in the title application or to a change in the use of the vehicle. For each block checked, write the correction after Correct to: Make of Vehicle, Body Type, Year, Seating Capacity, Unladen Weight, Fuel Type, Registered Gross Vehicle Weight, Registered Gross Combination Weight, Odometer (See Instructions on Reverse side), or Other.
Verification of Weight Rating, Inspection Mechanic Only
Verification of Weight Rating, Inspection Mechanic Only: enter the GVWR and the GCWR. The Inspection Mechanic signs and enters the Inspection Mechanic Number.
Number of axles and additional axle
Mark YES or NO for "Was additional axle installed?" Write the correct No. of Axles after "Correct to:". If additional axle was added, Form MV-426B must be completed.
Certification
Enter the Date and the Applicant's Telephone Number. In the box Signature of Co-Owner or Title of Authorized Person, a co-owner signs if there is one; if an authorized person signed for the owner, write that person's title.
By signing, the owner or authorized person certifies under penalty of law that all information is true and correct.
Other forms and situations
Name on a Pennsylvania Certificate of Title
To correct or change the name listed on a Pennsylvania Certificate of Title, use Form MV-41A.
Vehicle modified
If the vehicle has been modified in any manner, such as the addition or removal of axles, or you are requesting a change to the manufacturer weight specifications, Form MV-426B must be used.
A title is required and you do not have a title to provide
If a title is required as part of the correction and you do not have a title to provide, you must also submit Form MV-38O with the duplicate title fee along with this MV-41 for the correction and the fee for the correction. PennDOT will issue the duplicate title and attach it to the MV-41 to make the correction. Two title fees are required in this scenario.
Duplicate title or replacement of registration
If this application is processed, and the requested title, registration, or both are not received due to loss in the mail, the vehicle owner may apply for a free duplicate title on Form MV-38O, or for free replacement of registration by completing Form MV-50, Application for Replacement of Never Received Product, within 90 days of the date of original issuance.
The lienholder, if applicable, must use Form MV-38L, Application for Duplicate Title or to Record, Renew, Remove a Lien, or to Correct Lien Information by Lienholder, and a Pennsylvania Motor Vehicle Dealer must use Form MV-38D, Application for Duplicate Certificate of Title by a Pennsylvania Registered Dealer With a DIN#.
