Lienholder Name, Agent's Name and Date Received
Enter the Lienholder Name, then the Agent's Name and the Date Received.
Date of Lien, Amount of Lien, Delinquent Date and Possession Date
Enter the Date of Lien, Amount of Lien, Delinquent Date and Possession Date.
Vehicle Identification Number (VIN), Year, Make, Model and Body
Enter the Vehicle Identification Number (VIN), Year, Make, Model and Body.
Agent Signature and Date (MM/DD/YY)
The certification on page 1 is: "I certify, under penalty of perjury in the second degree, that the above Chattel Mortgage is in arrears and lienholder/agent is in possession of the vehicle."
Sign on the Agent Signature line and enter the Date (MM/DD/YY).
Certification of Extension of Chattel Mortgage: Vehicle Identification Number (VIN), Year, Make, Model and Body
Under Certification of Extension of Chattel Mortgage, enter the Vehicle Identification Number (VIN), Year, Make, Model and Body.
Certification of Extension of Chattel Mortgage: Date Received, Original Amount of Lien, County Filed In, Date Extension Filed, Amount of Secured Unpaid Debt and Lien Extended To
Enter the Date Received, Original Amount of Lien and County Filed In, then the Date Extension Filed, Amount of Secured Unpaid Debt and Lien Extended To.
Certification of Extension of Chattel Mortgage: Lienholder Name, Owner, Signature and Date (MM/DD/YY)
Enter the Lienholder Name and the Owner.
The certification on page 1 is: "I certify, under penalty of perjury in the second degree, that the above extension of Chattel Mortgage was filed in the appropriate county."
Sign on the Signature line and enter the Date (MM/DD/YY).
Floor Plan Declaration: Lienholder, Date and UCC Filing Number
Enter the Lienholder after "This is to declare that". The lienholder is the owner and holder of a note secured by a chattel mortgage executed on the Date you enter, with the UCC Filing Number.
Floor Plan Declaration: Dealer's Name and Dealer Number
Enter the Dealer's Name and Dealer Number of the dealer the chattel mortgage is against.
Floor Plan Declaration: Vehicle Identification Number (VIN), Year and Make
The following described motor vehicle is included in that agreement: enter the Vehicle Identification Number (VIN), Year and Make.
The Floor Plan Declaration declares that the note and chattel mortgage is a valid and existing lien against the vehicles held under agreement.
Floor Plan Declaration: Lienholder, Agent Signature and Date (MM/DD/YY)
The Floor Plan Declaration declares that the note and chattel mortgage is in default; in accordance with the provisions of the chattel mortgage, the undersigned has taken possession of the vehicle; and the vehicle is now in his/her possession.
The certification on page 3 is: "I certify, under penalty of perjury in the second degree, that the above facts are true and accurate to the best of my knowledge."
Enter the Lienholder, then sign on the Agent Signature line and enter the Date (MM/DD/YY).
More rules for Form DR 2393
DR 2393 has been combined to include the DR 2004 Floor Plan Declaration, the DR 2412 Statement of Repossession (Reverse side) and the DR 2417 Certificate of Extension of Chattel Mortgage (Reverse side).
Transferring the vehicle: a licensed Colorado dealer or not
Transferring the vehicle
If the lienholder is a licensed Colorado dealer, they may transfer this vehicle using a Colorado Dealer's Bill of Sale (DR 2407).
If the lienholder is not a licensed Colorado dealer, they must establish title in their name before transferring ownership.
Before you go
The Floor Plan Declaration must be accompanied by all documents of ownership; Manufacturer's Statement of Origin or Title properly assigned to defaulting dealer.
