5. Termination date: written revocation. Under TERMINATION DATE, initial the first line if you want the power of attorney to end when you have signed a written revocation. Choose one termination line only.

   

6. Termination date: set date or incapacity. Instead of the first line, you can initial the line that ends the power on a date you choose and fill in the date and the year. Or initial the line that ends it when you are determined to be incapacitated. The form defines incapacitated in the paragraph just below, so read it first.

   

7. Successor attorney-in-fact. On page 4, write your agent's name first. Then enter the name and address of the alternate agent who takes over if your first agent cannot serve.

   

8. Signed this day: date, signature and printed name. Fill in the day, the month and the year on the “Signed this” line. Then sign on the signature line and print your name under it.

   

9. Acknowledgment: county, date and officer. Page 5 holds the notary acknowledgment. Enter the county, the date, the officer's name and title, and your name as the person who appeared before the officer.

   

10. Officer signature and seal. The signature line above “(seal)” at the bottom of page 5 is for the notary or other officer. Leave it empty for them to sign and seal.
