We found no Hawaii statute that sets what a vehicle bill of sale must contain. In practice, a bill of sale is a written record of who sold what to whom, for how much and on what date. A clear record helps both sides if a question comes up later, including for tax purposes.
A complete Hawaii vehicle bill of sale usually shows:
Honolulu County adds one condition for the title. If the certificate of title is not released by the recorded owner, a notarized bill of sale must be provided. Our form has a notary acknowledgment on its last page for that purpose. The county page also says that if there are joint owners, both owners must sign the form.[1]

On page 3, read this part before you sign. It has nothing to fill in. It covers the ownership statement, the as-is terms and the entire agreement.
Step 6: Signatures
Each party signs, prints a name and adds the date. The form also has a witness line with a printed name and date.
Step 7: Acknowledgment
If you need a notary, sign in front of the notary on page 4. The county, the date, the officer’s name and title, and the name of the person appearing are filled in there. The officer then signs and adds the seal.
