The AFP Police Check form is the standard document for requesting a National Police Check (NPC) from the Australian Federal Police. The AFP processes checks for individuals across Australia and for some international applications.

There are two check types. A standard name check is suitable for most employment and volunteer roles. A name and fingerprint check is required for roles specified by law, including aged care, child care, firearms licensing, and certain government positions. If you also need a background check consent form for your employer, you can download one on FormsPal.

To complete the AFP police check form, you must provide 100 points of identification. Accepted documents include an Australian passport, birth certificate, driver licence, and Medicare card. All identity documents must be current. You must also list your residential history for the past 5 years, including dates and addresses. Payment is due at the time of submission. The AFP accepts credit and debit cards, bank cheques, and money orders. Processing time for a standard name check is typically 5 to 15 business days.
