How to complete the Signature and Tally Record
Open the PDF using the button above, then fill in each section following the steps below.
- Shipper information. Enter the full name, address, and unit identification of the shipping organization. Include the contract number if the shipment uses a commercial carrier.
- Cargo tally. List every item being transferred. Record the national stock number (NSN), item description, quantity, and unit of issue for each line in the tally section.
- Carrier acknowledgment. The carrier signs and dates the form when accepting custody, confirming the tally matches the items received.
- Consignee receipt. The receiving organization signs upon delivery, noting any discrepancies between the tally and the actual items received.
- Retention. Each party (shipper, carrier, consignee) keeps a signed copy for their records in accordance with DoD Regulation 4500.9-R.
