It captures key details needed to calculate reimbursement, including the origin and destination of the move, estimated weight of household goods, any advance operating allowance request (up to 60% of estimated reimbursement), and certification of actual expenses after the move is complete. All branches of the U.S. Armed Forces are covered, including Army, Navy, Air Force, Marine Corps, Coast Guard, National Guard, and Reserve members on qualifying orders.

Before completing the application, a mandatory pre-move counseling session with the TMO or Personal Property Office is required. Accurate weighing of household goods is central to the reimbursement process. Service members must obtain certified weight tickets from a certified commercial scale facility before loading their vehicle and again after unloading at the destination. The net weight of goods moved determines the final reimbursement calculation. If the actual cost of the move is lower than the government's estimated cost, the service member keeps the difference as an incentive payment under the PPM program.

The completed application, along with weight tickets and all supporting documentation, must be submitted to the transportation office within the required timeframe after the move is completed. Failure to submit documentation on time may result in reduced or forfeited reimbursement.
