“Guarantor Rider” Form
There are cases when renters do not have a long credit history. However, such renters should guarantee their credibility when applying to lease property in New York. To do so, they sign an additional document called a “Guarantor Rider” form. In the form, the renter names a person that may vouch for them. This form has to be signed by a notary public.
A rental application is accompanied by a fee to cover the security check conducted by the landlord. New York laws limit this amount to a maximum of $20 (Article 7, Section 238-A of the Consolidated Laws of New York).
Also, the landlord has to issue the applicant a receipt as proof.

2. Insert the Property Description
The first section of the form is dedicated to the leased property. Here, you should specify the property type (whether it is an apartment, a condominium, or any other type), its floor area in square feet, the number of bedrooms, and the rent charged per month. Below that, you should choose your preferences regarding pets, permission to smoke, and parking options.
3. Add Tenancy Period
Here, you need to insert the period you want to rent space and the first day of the tenancy.
4. Fill out the Applicant’s Information
Enter the applicant’s details, including the name, date of birth (DOB in the form), and social security number ( SSN), driving license number, email address, and telephone number.
If there are other potential occupants, describe them. Also, state whether you have pets and vehicles. If you have ever filed for bankruptcy, you should reveal this information here.
5. Reveal Employment Details
In this section, describe details of your employment, current and previous. In both cases, include the name of the employer, salary, and the employment term.
6. Add Residence Information
Below the employment details, give details about your residence, current and previous. This includes the type and size of the property, reasons for moving, addresses, periods of the lease, and information about former landlords.
7. Insert Personal References
Here, you should add the names and contacts of your personal references and how you are related.
8. Provide Information about Finances
This section is tied to your bank account(s) information. Fill in the blank lines with the bank name, your account number and type, and other required details.
9. Read the “Consent and Acknowledgment” Section
Below the banking details, there is a “Consent and Acknowledgment” text that every applicant should read. If you accept, affix your initials.
10. Sign the Form
After reading the acknowledgment, sign the application form and write the date.
11. Read the Consumer’s Disclosure
The last page of the form is the Consumer’s Disclosure. This disclosure allows the landlord to analyze the applicant’s information.
