A Utah bill of sale is a signed document that records a sale. It names the people on each side, describes the item that changes hands and states the price. Each side keeps a copy, so both have the same record of what was agreed.

The steps below follow our Utah motor vehicle bill of sale template, from the date at the top to the acknowledgment on page 4. Fill in each part in the order the form shows it. You can create the form online, print it and write it by hand, or fill it in with our online editor.
Step 1: Date
Write the date on the DATE line at the top of page 1.
Step 2: Seller’s Information
In Section 1, fill in the seller’s name, street address, city, state, zip code, phone number and email.
Step 3: Buyer’s Information
In Section 2, enter the same five details for the buyer.
Step 4: Motor Vehicle Description
In Section 3, write both names in the sentence at the top. Then fill in these fields:

Step 5: Purchase Date and Price
On page 2, in Section 4, enter the purchase date and the total purchase price in U.S. dollars.
Step 6: How the Price Is Paid
Check one arrangement: all at once, a trade-in, a deposit with a balance date, or a gift. For a trade-in, add the buyer’s car details. Then select one method: cash, check (with the check number), cashier’s check or money order.
Step 7: Taxes
Check whether all applicable taxes are included in the purchase price or are not included.
Step 8: Miscellaneous Provisions
On page 3, read Section 5 before you sign. This part has nothing to fill in. It holds the seller’s statement of ownership and no liens, and the as-is terms of the sale.
Step 9: Signatures
In Section 6, the seller and the buyer each sign, date and print their name. The form also has a Witness line.
Step 10: Acknowledgment
To have a notary complete the form, sign before the notary in the acknowledgment on page 4. The notary fills in the county, date, name, title, signature and seal.
When everyone has signed, make a copy of the signed bill of sale for each side. The buyer can take one to the DMV, and the seller keeps one as the record of the sale.
After a private sale, the buyer takes the paperwork to the DMV to put the car in their name. The title details entered must match the state’s records exactly.[2] Copy those details from the title, not from memory.
There is no 30-day window after a purchase. The buyer must obtain a new registration before driving the transferred vehicle on a highway or letting anyone else drive it (Section 41-1a-703).[3]
The DMV says you may take a bill of sale to the DMV, pay the title and registration fees and obtain a temporary permit so that you may use the vehicle while you wait for the title.[2]
