Name of Vessel, Voyage No. and IMO No.
Enter the Page No., Voyage No., IMO No., Nationality of Ship and Name of Vessel.
Time of Departure, Port of Discharge and Name of Master
Enter the Time of Departure from Port of Loading (Zulu), the Date of Departure from Port of Loading, the Port of Discharge, the Last Foreign Port Before U.S. and the Name of Master.
Shipper, Bill of Lading and Marks & Nos.
Enter the Shipper (SH), Consignee (CO) and Notify address (NF).
Enter the Bill of Lading No.
Enter the Marks & Nos. (MN), Container Nos. (CN) and Seal Nos. (SN).
Description of Goods, Gross Wt. and Measurement
Enter the No. & Kind of Packages and the Description of Goods; for Hazardous Materials you Must Provide UN Code.
Enter the Gross Wt. (lb. or kg.).
Enter the Measurement (per HTS).
First Port and Foreign Port Where Cargo is Laden
Enter the First Port/Place Where Carrier Takes Possession of Cargo.
Enter the Foreign Port Where Cargo is Laden on Board.
