In GOODS DELIVERED FROM, enter the Pier, Station, Airport, etc.
Under GOODS DELIVERED TO, enter the Street Address, the City, the State and the Zip Code.
Enter the DATE OF ENTRY and the DATE OF ARRIVAL.
Name and address of consignee, air waybill or bill of lading no. and importing carrier
Under NAME AND ADDRESS OF CONSIGNEE, enter the Last Name, the First Name and M.I., the Street Address, the City, the State and the Zip Code.
Enter the AIR WAYBILL OR BILL OF LADING NO.
Enter the NAME OF IMPORTING CARRIER. If Airline, give Airline and Flt. No.
Lien, lien satisfied and entry, sale, destroyed
Enter the LIEN NO., the DATE OF LIEN and the LIEN FILED BY.
Enter the DATE under LIEN SATISFIED (19 USC 1564).
Under LIEN SATISFIED (19 USC 1564), mark Entry, Sale or Destroyed.
Marks and numbers, description of merchandise and condition, weights
Enter the MARKS AND NUMBERS, the CONT. NUMBER, PIECE COUNT, the DESCRIPTION OF MERCHANDISE and the CONDITION, WEIGHTS, ETC.
Total no. of packages / units, signature of consignee and condition
Enter the TOTAL NO. OF PACKAGES / UNITS.
The consignee signs the SIGNATURE OF CONSIGNEE.
After Delivered in good condition except as noted and after Received in good condition except as noted, write what is noted.
Note the quantity discrepancies in QUANTITY DISCREPANCIES NOTE HERE.
Vehicle license plate number, custodial bond number, C.H. license number, truck number and company name
Enter the VEHICLE LICENSE PLATE NUMBER and the CUSTODIAL BOND NUMBER OF DELIVERING PARTY (19 CFR 113.63).
Enter the C.H. LICENSE NUMBER, the TRUCK NUMBER and the COMPANY NAME.
Signature of Cartman, Lighterman or Common Carrier, CBP officers and proprietors
The Cartman, Lighterman or Common Carrier signs in ink.
Enter the DELIVERING CBP OFFICER with the DATE and TIME, and the RECEIVING CBP OFFICER with the DATE and TIME.
Enter the DELIVERING PROPRIETOR and the RECEIVING PROPRIETOR.
