Filling out a crew time card requires careful attention to detail, as payroll accuracy depends on every field being completed correctly. The steps below explain how to fill out the form using our online PDF tool, and what each section of the form is designed to capture.
Step 1: Open the form. Click the orange "Get Form Here" button on this page to open the time card in the online editor. The form loads directly in your browser and no additional software is required to view, fill out, or submit the document. The editor supports most desktop and mobile browsers.
Step 2: Enter employer and job information. In the top section of the form, provide the employer's full name and address, the production job name and number, the union contract that governs your pay, and your occupation or job title. These fields identify the production and establish the pay agreement under which your hours will be calculated. Non-union crew members should note that in the contract field.
Step 3: Record hours and compensation. Fill in the work dates, daily call time, meal break start and end times, wrap time, and total hours worked each day of the production week. Enter your rate of pay and any additional compensation such as box rental, car allowance, mileage, or per diem. The form calculates gross earnings from these entries, so make sure every day's hours are recorded accurately before moving to the totals section.
Step 4: Review all fields before submitting. After completing the hours and compensation sections, review every entry carefully. Confirm that your social security number is correct in the employee section, that all work dates match your call sheet, and that the pay rate matches your deal memo. California crew members should read the work-related injury notice at the bottom before signing. Any errors in hours or rates can delay your payroll payment.
Step 5: Submit the completed form. Click the "Done" button to finalize the document and download the completed PDF. Submit the original to your production's payroll department by the required weekly deadline. Most productions require time cards to be submitted within 24 hours of the last day of the work week. Keep at least 2 copies of the signed time card for your own records in case of any payroll disputes or audit requests later in the production.
Tips for accurate time card completion: Always use your legal name and social security number, not a nickname or abbreviation. Record meal breaks separately from work hours, as missed or shortened meal breaks may trigger additional penalty payments under your union contract. If you are entitled to overtime, double-check that the hours are recorded in the correct overtime column. Contact your payroll department or unit production manager if you are unsure how to complete any section of the form.
