Revocation power of attorney is a form you can use to cancel your previous POAs. Under Illinois law, if you have the capacity to do so, you can revoke a power of attorney at any time and in any manner communicated to your agent or to any other person related to its subject matter.[2] Revoking a health care power of attorney is governed by Section 4-6 of the Illinois Power of Attorney Act.[2] The revocation form should be made for every POA form that the principal wants to eliminate. Illinois law does not require you to send a separate revocation form to your agents.[2]
